Informed Consent and Waiver of Liability Disclaimer

Upon enrollment in programs or services, or upon use or purchase of a product offered by Mindful Heart Wellness, LLC I agree to the following: I have consulted with my healthcare provider and have been cleared to participate in all activities. I take full responsibility for moderating my activities or lack thereof to honor my well-being. I hereby release Mindful Heart Wellness, LLC, and my teacher/practitioner/coach/trainer from any liability, now or in the future, for any injury or illness occurring during or after my participation or use of a Mindful Heart Wellness product or service. At no point should any information or suggestions provided by or through Mindful Heart Wellness be construed as medical, nutritional, or mental health advice, treatment, or diagnosis. This is the entire agreement of the involved parties and reflects a complete understanding of the parties concerning the subject matter. This agreement supersedes all prior written and oral representations.
